Some women will barely notice when the device is placed, she said, but others report the worst pain they will ever feel.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clzo9sm6r00003b5vlzotbtlu@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The devices, which are placed within the uterus to prevent pregnancy, are the&nbsp;third most common&nbsp;form of birth control in the US. Still, some patients have come to her after being dismissed by other providers \u2014 told to \u201ccalm down\u201d or that their pain is \u201cnot that bad\u201d or even \u201cyou\u2019re being dramatic.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznmbktd00033b6lpxp6rn2w@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            In new contraceptive guidance published this week, the US Centers for Disease Control and Prevention gave&nbsp;updated recommendations&nbsp;for clinicians on how to help manage the pain some people may have when the devices, known as IUDs, are inserted. Lidocaine \u201cmight be useful for reducing patient pain\u201d when injected as a local anesthetic or applied topically as a numbing gel, cream or spray, the CDC said in the update, the first since 2016.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb00083b5veq5bttju@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The CDC also recommends doctors inform all patients about potential pain and personalize IUD placement and pain management plans for each individual.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb00093b5vml2eqjp9@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The individualized and patient-centered language is a large shift from the 2016 guidelines, which were less specific and less detailed, according to Dr. Tessa Madden, professor of obstetrics, gynecology and reproductive sciences at the Yale School of Medicine.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhoz5q00003b5vwq8g7w55@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Madden\u2019s research on the use of lidocaine was referenced in the updated guidelines. However, she noted, her research is nearly 12 years old \u2014 highlighting the longstanding challenge in improving pain management during the procedure.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clzobji7p00123b5v3ugacpd6@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            In a 2019 survey of about 1,000 family planning providers, less than 5% reported using a lidocaine injection during IUD insertion. Instead, they more frequently suggested ibuprofen, which research has shown does not reduce insertion pain.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clzobx9t300143b5vjn0t5unt@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cSaying to take ibuprofen is the bare minimum, and it\u2019s not enough,\u201d said Edwards-Latchu, whose campus health clinic at University of North Carolina Chapel Hill offers lidocaine gel and injections, heating pads, Valium and the option to bring a supporting person. The clinic is also experimenting with umbilical vibration devices that might reduce discomfort during the procedure.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000a3b5v06mb5858@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The&nbsp;procedure to insert an IUD&nbsp;takes about 15 minutes or less. A health-care provider performs a pelvic exam and then uses a speculum to widen the vagina, through which they place the device within the uterus.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000b3b5v54ttvl3t@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cAny one of those steps can be uncomfortable for people\u2014from the pelvic examination to the IUD insertion\u2014although it\u2019s typically the insertion that\u2019s the most uncomfortable, when they experience the highest level of pain,\u201d said Dr. Beverly Gray, an associate professor of obstetrics and gynecology at Duke University and an obstetrician and gynecologist at Duke Health.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clzo96mll00003b5vvfs7cone@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Cramping and other uncomfortable or painful sensations can occur during and after insertion, as the procedure involves using pointed forceps called a tenaculum to stabilize the cervix and passing the intrauterine device through the narrow cervical canal.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000c3b5v6yfot2h5@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cPatients have clearly spoken out about their traumatic or painful experiences,\u201d Gray added. \u201cThese guidelines highlight the importance of discussing pain management and being frank about the spectrum of experiences that people might have.\u201d    <\/p>\n<h2 class=\"subheader\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/clznhpjzp00023b5vicjrch3a@published\" data-component-name=\"subheader\" id=\"my-pain-is-not-being-taken-seriously\" data-article-gutter=\"true\">        \u2018My pain is not being taken seriously\u2019<\/h2>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000e3b5vdejlqg50@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Female pain has long been underrecognized and undertreated.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000f3b5vkr4dthxb@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cWomen have been saying for decades, \u2018my pain is not being taken seriously,\u201d Edwards-Latchu said. \u201cIt feels like screaming into the void.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000g3b5v46jfs7m2@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            A 2021 study found that pain in female patients is consistently underestimated due to gender stereotypes, and women are judged to benefit less from pain medicine than men, despite equal likelihood of seeking care.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clzoaj6x0000q3b5vy2923emj@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cIn medicine we historically have not done a very good job of taking women\u2019s pain, in particular, seriously,\u201d added Madden.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clzoaem1t000g3b5v4mhvy14a@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Edwards-Latchu described a female patient whose appendix ruptured after her abdominal pain was dismissed as menstrual cramps, pointing to a broader issue of women\u2019s pain being mismanaged. Research echoes this, showing women in emergency rooms with similar abdominal pain scores to men waited longer and were less likely to receive pain relief.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clzoaen7o000i3b5v2eard77f@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            This issue also extends to IUD insertion, where studies have shown patients report significantly higher pain levels than providers perceive.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000m3b5vdznr99wr@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            There is no guaranteed way to predict an individual\u2019s response to an IUD insertion, according to Madden, who said that some of her patients have high levels of pain while others experience \u201cvery little.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000n3b5vssskuttk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            However, increased pain during the procedure is more likely if the patient hasn\u2019t had a vaginal birth, has a history of painful periods or has experienced trauma.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000o3b5v6srsoerk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Some research also suggests that&nbsp;anxiety is associated with worse pain&nbsp;across various medical procedures.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000p3b5vkc71n1k0@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cThere is a variety of experiences that people have. It\u2019s sometimes hard to predict who will have an easy experience with insertion and who will have a more challenging experience,\u201d Gray added. \u201cUnderstanding that is important.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000t3b5vqnzw2iuh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            A 2014&nbsp;study&nbsp;of 109 IUD recipients found that 78% reported pain ranging from moderate to severe upon insertion. That range is clear on TikTok, where numerous women have shared live videos from the exam table as their IUDs were inserted.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000s3b5veoz0qve1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cIUD insertion is the worst pain imaginable,\u201d wrote one user who&nbsp;showed&nbsp;herself writhing in pain.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhy6n100013b6llium97ui@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cThis was the most excruciating pain I\u2019ve ever experienced,\u201d another&nbsp;shared.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhyadj00033b6ltvbjohdf@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            In both videos, the health-care providers inserting the IUDs could be heard telling their patients to expect pressure or a \u201cpinch.\u201d    <\/p>\n<h2 class=\"subheader\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/clzoam70h000s3b5vtpg1iztx@published\" data-component-name=\"subheader\" id=\"social-media-shows-part-of-the-picture\" data-article-gutter=\"true\">        Social media shows part of the picture<\/h2>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000u3b5v7ztoi719@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cIt\u2019s heartbreaking,\u201d Edwards-Latchu said of the videos on her social media feed. \u201cThe first thing that I think is, \u2018I doubt they were given anything for their pain,\u2019 and to me, that is upsetting.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000v3b5vpfe2x4s3@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The videos bring awareness to the potential pain, she said, adding that it\u2019s important \u201cto know about the negative experiences so that something changes.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000w3b5vnqyvrbdf@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            However, according to Madden, the videos can also dissuade viewers from considering a contraceptive option that could be a viable option for them.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmb000x3b5vb2cqyd4m@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cPatients come into the office talking about videos that they\u2019ve seen on TikTok or Instagram, which is where a lot of times patients are getting their information from,\u201d she said. \u201cSeeing a video like that could be a significant deterrent.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhz59i000b3b5vcsqg15ge@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Intrauterine devices have had a tumultuous history. Notably, the&nbsp;Dalkon Shield&nbsp;in the 1970s caused cases of severe infection and other complications, casting a long shadow over the safety of IUDs. However, modern IUDs have been proven to be safe and effective, and are used by over 10% of women aged 15 to 49, according to CDC data from 2017-2019.<strong> <\/strong>    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznuf28o00023b5vcb6paw5r@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            They can last for up to 10 years or more and are 20 times better at preventing unintended pregnancies than birth control pills and other short-term contraceptive methods, according to a 2012 study.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmc000z3b5vbag6yglo@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201c[An IUD] is a highly effective method that many patients are very satisfied with,\u201d Madden added. \u201cFor some patients, the concern about pain with insertion is the reason that they\u2019re not using [it].    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznv08ef000d3b5vieqe2rst@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cIf patients feel like we\u2019re addressing their concerns about the pain, and taking the concerns seriously, then that might increase people\u2019s willingness to use the method.\u201d    <\/p>\n<h2 class=\"subheader\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/clznutokl000a3b5vzynrg3k5@published\" data-component-name=\"subheader\" id=\"what-to-ask-your-doctor\" data-article-gutter=\"true\">        What to ask your doctor<\/h2>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznj0lku00003b5va4ic0ll7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The updated guidelines come as there\u2019s a rise in demand for contraception after the reversal of Roe v. Wade in 2022.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clznhjmmc00103b5vgizuzgfs@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Edwards-Latchu explained that each year around graduation, her campus health clinic sees a surge of students seeking IUDs.
